Social Worker/Senior Practitioner, CHC Team, Herefordshire  
Herefordshire Adult Social Care is looking for an experienced CHC Senior Practitioner to undertake Joint Assessments / Reviews with our ICB to assess eligibility for CHC and Joint Funding.  Must have a strong working knowledge of eligibility and process under the 2022 National Framework for NHS Continuing Healthcare and the Care Act 2014. Experience of undertaking CHC Checklists, Decision Support Tools and undertaking challenges and disputes regarding CHC Eligibility and Joint Funding with the ICB.  ng part of a duty rota and sharing this responsibility with colleagues.
