Adult Social Worker, LD Team, Lewisham 
Up to £28-32ph 
This role is within a project team called the progression and next steps team the team sit within learning disability service. The primary role is to complete reassessments of people in residential care settings with a view to stepping people down into less restrictive care, such as supported living or shared lives, equally the project is ensuing that people within care settings have the opportunity to be upskilled to reduce reliance on commissioned care. The candidate needs to be experienced in undertaking strength based assessments, evidence based support, negotiating costs.  Must have experience in learning disability social work team, good assessment analytical skills, good communication skills, good negotiation skills, legally literate able to undertake, MCA, BI , CoP work. Savings targets are attached to the project and candidate will be expected to deliver on savings 
Candidate need to have a background in working with adults with LD, ADHD, Autism. Need to have completed ASYE , 2 years post qualifying. Must have good skill set including, strength based assessment, reviews, legal literacy, must have good working knowledge of legislation including MCA and DoLS , Safeguarding, Care Act. Equality Act. All assessments and Reviews are undertaken face to face. SW will have access to in house training and wider team learning events for CPD.
