Adult Social Worker, At Home Service, Lambeth  

Based at Elm Court Health Centre, SE27 9AQ.  

Pay rate: £30–£32 per hour (depending on experience)  
Position type: Full-time (35 hours per week, Monday–Friday) Working pattern: Office-based five days per week during triage/duty weeks (approximately one week per month) Hybrid working at other times, with a minimum of three office-based days per week 
Key requirements:** Social Work England registration and enhanced DBS Strong assessment, risk assessment, and safeguarding skills Experience managing complex caseloads within a fast-paced team Excellent communication skills and a strong commitment to Equality, Diversity, and Inclusion Relevant experience in either front door, hospital discharge, Reablement or intermediate care services teams We are a supportive management team and are looking for an experienced practitioner who can hit the ground running and has a good understanding of working within a fast-paced, short-term intervention service.
