Hospital Discharge Social Worker, Bournemouth 
Locum Social Worker – Hospital & Discharge to Assess Team  
We are seeking an experienced and motivated Social Worker to join our busy Hospital Social Work Team at University Hospitals Dorset, working across Royal Bournemouth Hospital and Poole Hospital. 
This is a varied and dynamic role which may involve working within the acute hospital setting as well as supporting individuals following discharge into the community, including Discharge to Assess (D2A) follow‑up work. The post holder should therefore be confident working across both hospital and community environments and adapting to service demand. 
Hybrid working.  
£32.33 per hour
