 Vitalis are recruiting for an experienced Social Worker to work in a Transitions Team in Manchester City Council. 
 
MCC are in need of a social worker for the Transitions team - Must be social work qualified and registered with social work england. We are looking for adult social workers to complete care act assessments, support planning and review in a preparing for adulthood service.
Essential car user
Lone working required
Expected to be in the office at least 2 days a week
 
There is plenty of hybrid working available.
