Qualified Social Worker – Adults Team
📍 Location: Cheshire
💰 Pay Rate: £30 per hour

🕐 Contract Type: Agency | Hybrid working available
Are you a Qualified Social Worker with a passion for supporting adults to live safely, independently and with dignity? We're looking for experienced practitioners to join a dedicated Adult Services Team covering both community work and hospital discharge.
This is a fantastic opportunity to work in a supportive, values-led environment that champions best practice and focuses on meaningful, person-centred outcomes.

🌟 What You’ll Be Doing:

Carrying out strengths-based assessments and supporting timely, safe discharges from hospital.
Managing a varied caseload, including complex care planning and safeguarding.
Working closely with health professionals, families and voluntary services to provide joined-up support.
Promoting independence, dignity, and wellbeing at every step.

👤 We’re Looking For:

A Qualified Social Worker registered with Social Work England.
4+ years’ post-qualified experience in adult social work settings.
Strong knowledge of the Care Act 2014 and relevant adult safeguarding procedures.
Skilled in building effective partnerships across health and social care.
Confident, reflective, and committed to continuous improvement.

✅ You’ll Need:

A relevant social work qualification (DipSW, BA, MA, CQSW, etc.)
Excellent communication and case recording skills.
A proactive, flexible mindset – and the ability to hit the ground running.
